This document presents a prototype Individual Transition Plan (ITP) process and format for students receiving special education services. Following an overview of the role of transition, a section on the ITP process discusses pre-meeting activities, conducting an ITP meeting, monitoring and evaluating the ITP meeting, and a rationale for the selection of this ITP process prototype. A transition planning guide explains to students, families, and advocates the purpose of an ITP and the events that occur at a transition plan meeting. The following section provides a form for use in the ITP process, illustrates use of the form by including a sample completed form, and provides a rationale for selection of this ITP format. Recommendations suggest future activities to identify and implement the valid components of the prototype ITP, such as formation of an ITP advisory committee, development of training programs, and broadening of the ITP process to include "at risk" youth. Appendices include a parent questionnaire, field input questionnaire, the dimensions of the ITP formats surveyed, a summary of the prototype reviewers' comments, and examples of supplemental forms.
