Intellectual functioning that is more than four standard deviations below the mean (usually below 40 IQ range), concurrent with adaptive deficiencies -- individuals with a severe intellectual disability require continual supervision and care. Prior to 2015, "Severe Mental Retardation" was used to index this concept.
