This publication consists mainly of sample forms used by public school districts throughout the U.S. at the time an employee leaves his or her position. Included are eight different questionnaires and similar forms for use in conducting exit interviews or otherwise gathering information on terminating employees. Also included are reprints of two journal articles on why and how to conduct exit interviews. These include "Separation Interviews," from the September 1977 issue of Personnel Journal and "The Case for Exit Interviews," from the June 1977 issue of the American Association of School Personnel Administrators Bulletin.
