To help define the role of instructional television in arts education as background information for AIT's Art Project, a sample of 12 state and three provincial level arts educators were asked to complete a questionnaire. Sixteen open-ended items were divided into three categories--curriculum, students, and teachers; some called for quantifiable responses, while others required narrative, qualitative responses. Individual responses from the 12 completed questionnaires are reported for each question. Appendices include the names and addresses of the educators contacted for the survey, names of individuals who were notified that the survey was to be conducted, and a copy of the State Arts Education Questionnaire and cover letter.
