Described is the social service delivery model of the Ochlocknee Project, a community based program for rural multiply handicapped preschoolers. Included is a job description for a staff social worker and detailed are objectives and procedures for three social service goals: identification and location of children with multiple handicaps; placement into appropriate educational and therapeutic settings; and provision of an individualized parent program for each family. A final section considers planning family objectives. Ten sample forms and a general resource guide are included in the appendix. (CL)
