The Case Management Model: Trainer's Guide. Volume III.

This manual is intended to serve as a trainer's curriculum guide on the case management model for social service workers. It is divided into seven sections, each of which focuses on one of the basic requirements of the case management process. The seven process requirements examined are: (1) evaluation of service request or need, (2) eligibility determination, (3) planning of service objectives, (4) arrangements for linking clients with service providers, (5) provision of services, (6) overseeing the case, and (7) recordkeeping. Each section contains a content outline of the major purpose and procedures of each requirement, some key points to be emphasized about case management during training, and references and training recommendations. Included among the training recommendations are questions to be raised and suggestions on training methods, the use of accompanying videotapes and other support materials. The training issues and suggestions covering overseeing and recording are amplified in Appendix A. Appendix B provides the case management flow chart and system functions. Appendix C is a list of selected publications thought to be relevant to case management training. A guide to the accompanying video tapes for case management training is included in Appendix D.
