Collaborative Transition Planning Systems in Los Angeles, County.

School districts and community agencies located within Los Angeles County (California) were surveyed to determine the level of transition planning from school to the world of work and community living for "students at risk." Twenty-five school districts and 10 community agencies/private schools receiving supported employment funds are profiled in this document. A comparison matrix presents an overall view of the programs, separately for public school districts and private school/adult agencies. The matrix provides information on demographics, services, transition team operational procedures, team membership and selection, parent practices and procedures, systems planning, and subjective/objective evaluation. In addition, there is a narrative describing the unique features of each respondent, focusing on its referral procedures, transition team operation, parental involvement, agency referrals, and interagency collaboration. Appendices, which comprise over half of the volume, contain materials received from survey participants, including samples of questionnaires, progress reports, curriculum materials for teaching pre-employment skills training and community transition training, team action plans, community resource directories, and other items.
