Universal screening and referral are crucial for maternal, infant, and early childhood home visiting programs to improve maternal, infant, and early childhood outcomes. The purpose of this article is to describe a county-wide, centralized intake and referral system. The authors report results and lessons learned for reducing service duplication and gaps. These include the need for high collaboration, universal screening that makes use of established screening measures with adequate psychometric integrity, targeted referral support based on established screening measures, and staff who are well-trained and knowledgeable about the enrollment criteria of multiple and diverse agencies that serve families with young children.
