Training Tomorrow's Doctors: The Medical Education Mission of Academic Health Centers. A Report of the Commonwealth Fund Task Force on Academic Health Centers.

This report, fifth of a series on Academic Health Centers (AHCs), addresses the fundamental rationale of such centers: the education of the health care workforce. None of the missions of the 125 AHCs in the United States, medical schools and their closely affiliated hospitals and physician groups, is more important than the education of physicians. This report begins with sections that outline the organization and size of the medical education enterprise, how medical education is financed, the unique role of AHCs in the medical education process, and the importance of medical education to various individuals and organizations in U.S. society. Also presented are the findings of the Task Force on Academic Health of the Commonwealth Fund on the nature and extent of the challenges facing AHCs and their educational missions and how AHCs have responded to these challenges. The next section presents a set of conclusions and guiding principles based on the findings. The final section contains recommendations grouped into those for AHCs, accreditation and similar groups, and public policy. Findings make it apparent that the content and quality of physician training will remain vital for the foreseeable future to the health of the U.S. population and the effective functioning of the U.S. health care system. The data available are not sufficient to judge the performance of AHCs in discharging their educational responsibilities beyond establishing a minimum level of competency. AHCs should be held accountable for their performance in educating U.S. physicians, and the extra clinical costs associated with the educational missions of AHCs should be borne broadly and fairly by the beneficiaries of the educational activities of these institutions.
