This student workbook and accompanying teacher's guide are designed to help children with attention deficit disorders (ADD). The Attention Deficit Accommodation Plan for Teaching (ADAPT) teacher planbook guides the teacher in evaluating the students' areas of difficulty. This evaluation is the basis for the design and implementation of classroom accommodations focused on facilitating academic performance and improving students' social adjustment and self-esteem. Forms found in the teacher planbook include an academic performance inventory that evaluates the attention, impulsiveness, motor activity, organization/planning, compliance, mood, social interaction, and academic skills of the student; an accommodation plan that lists different accommodations that may be made; a tracking form to evaluate the effectiveness of the chosen accommodations; and a medication effects rating scale. The teacher planbook includes information on Section 504 of the Rehabilitation Act of 1973 and instructions for using the daily record forms, self-monitoring activity sheets, and behavioral contract form contained in the student planbook. The forms in the student planbook are designed to help students plan their assignments and keep track of their daily performance. This planbook also contains worksheets to be used in conjunction with the accommodation plan designed by the teacher. (CR)
