This report presents recommendations that address needed changes and improvements in state policies, administrative practices, and school and community service programs, in relation to school-to-adult-life transition of young people with disabilities in Minnesota. Recommendations were derived from two sources: (1) reviews of state plans and reports, and federal and state legislation and policies; and (2) interviews with policymakers, professionals, individuals with disabilities, and family members throughout Minnesota. The report begins with a brief history of Minnesota transition policy. Sixty-five recommendations are then presented, within the categories of state level, community level, and individual and family recommendations. A framework for a general planning and implementation process is then presented, focusing on interagency and agency-specific steps. Appendixes list over 50 source documents and reports, identify commonly used acronyms, and summarize the report's recommendations. (JDD)
