Collaborative Community Based Transition Model. An Implementation Manual.

This manual is designed to help schools in the development, implementation, and evaluation of activities that assist individuals with special needs in making vertical transitions from school to postsecondary training or employment and horizontal transitions between the school, home, and community. It presents, in section 1, a model that accommodates the active collaboration of administration, agencies, special and vocational educators, clients, parents, guidance personnel, and employers. A figure presents the six-stage model designed to address the collaborative community-based transition process. Sections II, III, and IV address development of an internal steering committee, development of a transition core team, and the conduction of a community survey. Section V focuses on implementation of a community action plan, a phase that includes interagency collaboration, parent training, and school personnel training. In section VI, individualization of the transition process is discussed. The following issues that the core team must resolve during this phase, in collaboration with local school personnel, are covered: identification of schools and students for transition, in-school transitional planning, and development of the individual transition team. Section VII is a brief discussion of annual evaluation. Survey instruments and 23 references are appended. (YLB)
