A Guide to the Client Assessment Package.

This guide provides users of the Client Assessment Package (CAP)--a system of forms for recording the process of seeking and using information and assistance for educational improvement--with definitions, operational procedures, and instructions necessary to complete the forms accurately. The purposes of CAP are threefold: to help develop an efficient system for gathering data concerning educational information services; to process client data efficiently so that programmatic changes can be based on timely client feedback; and to identify the significant interrelationships among individual, organizational, informational, and service variables in order to improve program efforts. CAP provides the basis for recording client requests for information and assistance in five machine-readable forms: (1) service form, (2) process form, (3) linker form, (4) immediate feedback form, and (5) client assessment form. Tabulations of CAP data, designed to supplement present information service assessment procedures, will include client and request descriptions, as well as the extent of satisfaction with, and utilization of, services. A sample Client Assessment Package, identification numbers, glossary of terms, and an example using CAP are appended. (CWM)
