Angelini, Dennis E.; Murphy, John F.
One of a series of manuals developed by the Home and Community-Based Career Education Project, the publication presents the activities of the project's research and evaluation component, which continually informs the staff about project development and describes client characteristics, needs, decisions, and plans. Generic issues which must be resolved in order to integrate research and evaluation into the operation of human services programs were developed from an informational system designed to collect relevant data about: existing needs, defining goals and research questions, collecting data, reporting results, and staffing. Appended are samples of the data collection forms: intake interview form, initial interview recording form, interview record form, summary of call form, termination form, client reactions and evaluation form, and resource center questionnaire.
