The purpose of the project was to increase the effectiveness and efficiency of those rehabilitation facilities which were utilized by the Ohio Rehabilitation Services Commission (RSC) through the development, installation, and utilization of a program evaluation system and a management information system. The two systems were developed and presented to fifteen facilities for installation and implementation. Initial reports from both systems proved to be inaccurate and incomplete, and therefore the management information report was discontinued and a program evaluation questionnaire was designed for executive directors and daily administrators to determine how the program evaluation system could be improved. After review of the data received, it was evident that facilities were unsure how their program evaluation systems were going to impact upon their programs, services, and clients, and that they needed continued support with their program evaluation systems. Recommendations included that RSC consider funding a second program evaluation project. This report of the project includes sixteen common facility problems with program evaluation systems, as well as advantages and disadvantages of the system to the RSC, the facilities, and clients. Over one-half of the report is devoted to an appendix which includes guidelines for the program evaluation project, the program evaluation questionnaire, a management information system description, and source document sample forms.
