The survey instruments that are a part of an ongoing systematic process of information gathering at Delaware County Community College (DCCC) form the bulk of this manual. Designed to familiarize the faculty and staff with the process of survey use for decision-making, the intended populations and the possible applications of data are described. Each instrument is preceded by a definition of the intended population, the method of administration, and an explanation of sample data included to demonstrate 1976 use of the instrument. The questionnaires included are designed for (1) non-returning students, (2) graduate follow-ups, (3) alumni surveys, (4) exit interviews, (5) educational intent surveys, (6) course withdrawals, and (7) walk-off surveys for unofficial withdrawals. The instruments represent the first phase of participation by DCCC in a Student Information System based on Tex-SIS, a series of surveys developed by the Texas Education Agency. (RT)
