The National School Health Services Program Evaluation machine-readable data files (MRDF) are the result of a series of surveys conducted during the 1979-1980, 1980-1981, and 1981-1982 school years. The purpose of these surveys was to evaluate a demonstration health service program sponsored by the Robert Wood Johnson Foundation. The principal objective of this program was to make primary health care services more accessible to school-aged children, especially to those who have unmet health care needs. These files document the nature and scope of a wide range of health services provided by nurse practitioners, school health nurses, physicians, and health aides. The information includes: (1) records of communication between educators, health professionals, and parents; (2) the type, severity, and disposition of problems treated at schools (including referrals and the types of health care professionals involved); (3) nurse practitioners' findings from medical histories and physical examinations of students; and (4) data on individual health care episodes at the school, including unresolved problems. Data are supplied for school health programs at a total of 30 schools at six sites: (1) Commerce City, Colorado; (2) Craig, Colorado; (3) Newburgh, New York; (4) Kidder, North Dakota; (5) Rollette, North Dakota; and (6) Tooele, Utah. Information supplied by a survey of parents of children in participating schools includes data on health care sources and expenses for the child, plus data on specific medical problems and treatment. Basic demographic characteristics such as sex and race of the child, parent's educational background, and family income are also provided. A total of nine files were developed from the communications form, the health aide forms, the history and the physical forms, the parent questionnaires, the episode problems, the practitioner forms, the episode visits, and the link file. The data include such variables as: birth dates, ethnicity, health care expenses, health problems, illnesses, injuries, medical appointment delays, medical diagnoses, medical expenses, office visits (length and disposition), physical abnormalities, physical examinations, sicknesses, transportation time, and waiting periods. POPULATION: elementary school students (18,142) participating in the National School Health Services demonstration program. TYPE OF SURVEY: Sample survey. RESPONDENTS: Nurse practitioners, parents, physicians, school health aides, and school health nurses. METHODOLOGY: A series of multi-part forms were sent to 30 selected schools, were completed by designated persons regarding specific school health services and practices, and were mailed back.
