Community Pathways: Hospital-Based Services that Individualize Supports for Families and Children

Increasing numbers of infants and toddlers who were premature, had low birth weight, or experience chronic medical conditions are referred to early intervention services (Bernstein, Heimler, & Sasidharan, 1998). These young children often endure prolonged hospitalizations and are at risk for developmental disabilities by nature of their illnesses, injuries, or diagnoses (Boyce, Smith, & Casto, 1999). Further, these children are at risk for motor delays, sensory deficits, behavior difficulties, and delayed social and adaptive functioning (Hack & Taylor, 2000). Statistics indicate that infants who are born prematurely or small for gestational age are more likely to be born to indigent, low income, and/or teenage mothers (Von Der Pool, 1998). Parents who may experience compromised financial and/or emotional resources may have considerable need for emotional, informational, and financial support. The purpose of this article is to examine how early intervention services may be designed and implemented to provide this much needed support for families and their young children. Specifically, this article describes the components of an individualized system of care for families and infants through a hospital-based early intervention program designed to meet families' needs during children's hospitalization and transition home. The stories of two families provide a context for the discussion and description of implementation of the various components of the program. (Contains 1 figure, 3 tables, and 1 note.)
