Performed an analysis of the root causes of events involving graduate medical trainees that were recorded in hospital-based near-miss reporting systems. Classified root causes using the Eindhoven Classification Model, medical version. Found lack of knowledge and organizational causes at root of near-miss incidents. Concluded that root cause analysis along with a near-miss event reporting system can provide valuable information. (EV)
