The guide is intended to be a practical tool for screening and identifying Wisconsin students suspected of being emotionally disturbed (ED). It contains information on the legal requirements for the identification and placement of ED students as well as the definition of emotional disturbance and techniques for screening and evaluation. Sample copies of instruments, interview forms, multidisciplinary team (M-team) reports, and individualized education programs are included, along with a discussion of the roles of various professionals at different stages in the assessment process. The first chapter covers the rationale for ED screening, the goals of screening social-emotional behavior, and the screening process. The next chapter looks at the Wisconsin M-team procedural requirements, process, and reports. Chapter 3 presents the definition and criteria for emotional disturbance, while chapter 4 discusses principles of assessment and describes 47 behavioral checklists and rating scales. Chapter 5 presents sample M-team reports and corresponding individualized education programs (IEP) for both the elementary and secondary levels. An extensive appendix includes samples of: screening forms, observation forms, a teacher interview form, a school behavior rating form, an early childhood behavior rating form, a school intervention form, a student interview form, a parent interview and questionnaire form, and sample M-team and IEP data gathering forms.
