This volume is the second part of a report evaluating the North Carolina Alcohol Drug Education Traffic Schools (ADETS), established for the primary purpose of treating first offenders convicted of driving under the influence (DUI). These appendices include copies of legislation pertaining to the schools; a copy of the DMH 2604 referral form; an application for Alcohol and Drug Education Traffic School Instructor Certification; a summary of a training class schedule and topics addressed; ADETS operating standards; a copy of the curriculum training manual; classification of items from the Pre-Post Knowledge Questionnaire into five content areas, and a summary of item characteristics; questions for surveying target groups; questions asked of each target group; survey procedures; and a survey of Alcohol Traffic Schools.
