This annotated bibliography is designed to meet the needs of health planners, including nurse planners, educators, administrators, researchers, and practitioners involved with community health nursing programs. Abstracts of references are grouped in four sections. Section one includes references to documents which describe organizational models directed toward assuring continuity of patient care. Section two provides reference materials on new health units such as long-term residential centers, childbirth centers, and hospices with home care services. In section three, information is provided on manpower including expanding roles and practices of nurse practitioners in a variety of community settings and home health networks. The final section gives information on acceptable standards and criteria for evaluating the appropriateness of community health nursing practice. (JD)
