Concerned with aspects of the Uniform Migrant Student Medical Transfer Form of the Migrant Student Record Transfer System (MSRTS), this health provider's manual also details the minimal health services which migrant children should receive and recommends the procedure for efficient interchange of information between the Wisconsin State Migrant program and the participating schools throughout the country. This procedure centers around the utilization of data contained on the MSRTS, which shows in part that four categories of health services in Wisconsin's migrant education program tend to show an age-associated increase in the rates of reported health abnormalities among three age groups. It is stated that continuing attention to the four health service categories (vision screening, physical exam, dental screening, and audio-metric screening) can significantly strengthen the educational process for Wisconsin's migrant children. Following a short discussion of the above categories, goals for migrant child health, health services, and health education are enumerated. The Uniform Migrant Student Medical Transfer Form is explained in detail and medical computer data procedures are listed, as are the MSRTS health problem computer codes. Appendices include recommended maintenance/mailing procedures, sample forms, immunization schedule, and migrant child population list. (AN)
