This set of descriptive instructions explains and defines each of the 19 elements of the Chancellor's Office Course Reporting and Data Collection Form (CCC-456), with the intent of providing the completer with a sense of the philosophical basis of the element. CCC-456 requires such basic information as the course title, semester units, college code, and United States Office of Education code. More complex elements of the form include the following: (1) course classification by level within the educational sequence; (2) type of noncredit course as defined by California Administrative Code, Title 5; (3) special program identifier for courses that are part of a special program as defined by the Education Code, Title 5, or the Labor Code; (4) combined course activities field to identify course activities specified in the Education Code and Title 5; (5) primary method of course evaluation; (6) primary teaching materials used; (7) method of instruction, reflecting educational technology and the use of facilities, equipment, and aides; (8) the method of demonstration of need for the course; and (9) the three most important course objectives drawn from a taxonomy with four sections--general knowledge, specialized knowledge, skill development, and personal development.
