By providing access to samples of forms and other evaluation/management documentation, this compendium provides a working tool so that state projects can share methodologies for gathering data and applying it for both summative and formative evaluation purposes. This compendium may also serve to indicate procedural gaps in some projects, or to assist in starting new projects. It is arranged in five sections, as follows: (1) request forms (inquiry records, negotiation forms, and information requests); (2) feedback (satisfaction) forms; (3) linker (field agent) documentation forms; (4) impact or utilization forms; and (5) miscellaneous other documentation forms. Each section begins with a brief analytic overview of the class of materials which is followed by the actual forms and questionnaires which are arranged alphabetically by state. (Author/CTM)
